What Are Side Effects Associated with Using Captopril-Hydrochlorothiazide?
Common side effects of Captopril-Hydrochlorothiazide include:
- Cough,
- Mild itching,
- Rash,
- Joint pain,
- Numbness in the hands or feet,
- Decreased sense of taste, and
- Flushing (warmth, redness, or tingly feeling)
Serious side effects of Captopril-Hydrochlorothiazide include:
- Hives,
- Difficulty breathing,
- Swelling of the face, lips, tongue, or throat,
- Severe stomach pain,
- Lightheadedness,
- Eye pain,
- Vision problems,
- Little or no urinating,
- Chest pain,
- Pounding heartbeats,
- Fluttering in the chest,
- Swelling,
- Weight gain,
- Shortness of breath,
- Pale skin,
- Easy bruising,
- Unusual bleeding,
- Sudden weakness,
- Ill feeling,
- Fever,
- Chills,
- Sore throat,
- Mouth sores,
- Trouble swallowing,
- Nausea,
- Slow or unusual heart rate,
- Weakness,
- Loss of movement,
- Leg cramps,
- Constipation,
- Irregular heartbeats,
- Increased thirst or urination,
- Numbness or tingling,
- Muscle weakness or limp feeling,
- Headache,
- Confusion,
- Slurred speech,
- Severe weakness,
- Vomiting,
- Loss of coordination,
- Feeling unsteady,

- Burning in the eyes,
- Skin pain, and
- Red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling
Rare side effects of Captopril-Hydrochlorothiazide include:

Seek medical care or call 911 at once if you have the following serious side effects:
- Severe headache, confusion, slurred speech, arm or leg weakness, trouble walking, loss of coordination, feeling unsteady, very stiff muscles, high fever, profuse sweating, or tremors;
- Serious eye symptoms such as sudden vision loss, blurred vision, tunnel vision, eye pain or swelling, or seeing halos around lights;
- Serious heart symptoms include fast, irregular, or pounding heartbeats; fluttering in the chest; shortness of breath; sudden dizziness, lightheadedness, or passing out.

What Are the Dosages of Captopril-Hydrochlorothiazide?
Adult dosage
Tablet
- 25 mg/15 mg
- 25 mg/25 mg
- 50 mg/15 mg
- 50 mg/25 mg
Hypertension
Adult dosage
- Initial: 25 mg captopril/15 mg hydrochlorothiazide orally once a day; not to exceed 150 mg captopril/50 mg chlorothiazide
- Increase either or both components based on clinical response every 6 weeks

What Are Warnings and Precautions for Captopril-Hydrochlorothiazide?
Contraindications
- Hypersensitivity to either component or sulfonamides
- History of hereditary or idiopathic angioedema
- Coadministration of neprilysin inhibitors (.g, sacubitril) with ACE inhibitors may increase angioedema risk; do not administer ACE inhibitors within 36 hr of switching to or from sacubitril/valsartan
- Bilateral renal artery stenosis or anuria
- Do not coadminister aliskiren in patients with diabetes

Cautions
- Excessive hypotension if concomitant diuretics, hypovolemia, hyponatremia
- Risk of hyperkalemia, especially in patients with renal impairment, DM, or those taking concomitant K+-elevating drugs
- Dual blockade renin-angiotensin system with ARBs, ACE inhibitors, or aliskiren associated with increased risk for hypotension, hyperkalemia, and renal function changes (including acute renal failure) compared to monotherapy
- DM, fluid or electrolyte imbalance, hyperuricemia, or gout, SLE, liver disease, renal disease
- May aggravate digitalis toxicity
- Sensitivity reactions may occur with or without a history of allergy or asthma
- Aortic stenosis/ hypertrophic cardiomyopathy
- Biliary cirrhosis or biliary obstruction
- Myelosuppression
- Blood levels do not correlate with BP response
- Causes false positive urine acetone
- Risk of male sexual dysfunction
- Avoid concomitant use of lithium
- Acute transient myopia and acute angle-closure glaucoma have been reported, particularly with a history of sulfonamide or penicillin allergy (hydrochlorothiazide is a sulfonamide)
Pregnancy and Lactation
- Use with caution if the benefits outweigh the risks during the 1st trimester; Use in LIFE-THREATENING emergencies when no safer drug is available during 2nd and 3rd trimester
