What Is Cyclopentolate-Phenylephrine and How Does It Work?

Cyclopentolate-Phenylephrine is a combination medication used to dilate pupils in preparation for an eye exam.

What Are Side Effects Associated with Using Cyclopentolate-Phenylephrine?

Common side effects of Cyclopentolate-Phenylephrine include:

  • increased eye pressure
  • burning/irritation upon instillation
  • eye sensitivity to light
  • blurred vision
  • superficial punctate keratitis (eye redness, watery eyes, sensitivity to light, decreased vision)

Serious side effects of Cyclopentolate-Phenylephrine include:

  • blurred vision, tunnel vision, or seeing halos around lights
  • eye pain
  • fast heart rate, severe headache, pounding in the neck or ears, anxiety
  • fever
  • little or no urinating
  • flushing (warmth, redness, or tingly feeling)
  • severe constipation
  • weak or shallow breathing

Rare side effects of Cyclopentolate-Phenylephrine include:

  • none 

Seek medical care or call 911 at once if you have the following serious side effects:

  • Severe headache, confusion, slurred speech, arm or leg weakness, trouble walking, coordination loss, feeling unsteady, very stiff muscles, high fever, profuse sweating, or tremors
  • Serious eye symptoms such as sudden vision loss, blurred vision, tunnel vision, eye pain or swelling, or seeing halos around lights
  • Serious heart symptoms include fast, irregular, or pounding heartbeats; fluttering in the chest; shortness of breath; sudden dizziness, lightheadedness, or passing out

What Are the Dosages of Cyclopentolate-Phenylephrine?

Adult and pediatric dosage

Ophthalmic solution

  • 0.2%/1%

Mydriasis/cycloplegia diagnosis

Adult and pediatric dosage

  • 1-2 drops in the conjunctival sac every 5-10 minutes, not to exceed 3 times

What Are Warnings and Precautions for Cyclopentolate-Phenylephrine?

Contraindications

  • Untreated narrow-angle glaucoma
  • Hypersensitivity to any component

Cautions

  • Reduce dose if used within 21 days of monoamine oxidase inhibitors or tricyclic antidepressants
  • Wait 5 minutes between multiple drops (no more than three drops needed)
  • May cause false-normal tonometry readings, so tonometry should be performed before phenylephrine is administered
  • Restrict to short-term use (safety and efficacy of long-term use not established)
  • Use caution in Down syndrome (predisposed to angle-closure glaucoma)
  • Remove contact lenses before administration and wait 15 minutes before reinserting
  • The elderly may be predisposed to increased intraocular pressure
  • May result in psychotic reactions and behavioral disturbances in children 30-45 minutes after instillation
  • Minimize absorption by using one drop of solution per eye and applying pressure

Pregnancy and Lactation

  • Use with caution if the benefits outweigh the risks during pregnancy
  • Lactation
    • Unknown if excreted in breast milk, so use caution
