What Is Lidocaine and Prilocaine Topical and How Does It Work?

Lidocaine and Prilocaine Topical is a combination medication used to numb the skin or surfaces of the penis or vagina. This medicine is used to prepare you for minor surgery or medical procedures in these areas.

What Are Side Effects Associated with Using Lidocaine and Prilocaine Topical?

Common side effects of Lidocaine and Prilocaine Topical include:

  • mild burning where the medicine is applied;
  • itching, rash; or
  • changes in skin color at the application site.

Serious side effects of Lidocaine and Prilocaine Topical include:

  • hives, 
  • difficult breathing,
  • swelling of the face, lips, tongue, or throat,
  • severe burning, stinging, or irritation where the medicine was applied;
  • swelling or redness;
  • sudden dizziness or drowsiness after medicine is applied;
  • confusion, blurred vision, ringing in your ears;
  • bruising or purple appearance of the skin; or
  • unusual sensations of temperature.

Rare side effects of Lidocaine and Prilocaine Topical include:

  • none

Seek medical care or call 911 at once if you have the following serious side effects:

  • Severe headache, confusion, slurred speech, arm or leg weakness, trouble walking, loss of coordination, feeling unsteady, very stiff muscles, high fever, profuse sweating, or tremors;
  • Serious eye symptoms such as sudden vision loss, blurred vision, tunnel vision, eye pain or swelling, or seeing halos around lights;
  • Serious heart symptoms such as fast, irregular, or pounding heartbeats; fluttering in the chest; shortness of breath; sudden dizziness, lightheartedness, or passing out.

What Are Dosages of Lidocaine and Prilocaine Topical?

Adult and pediatric dosage

  • Cream (EMLA)
    • (2.5%/2.5%)/30 g
  • Dental gel (Oraqix)
    • 2.5%/2.5%
  • Minor dermal procedures (e.g., IV cannulation or venipuncture): Apply 2.5 g (1/2 of the 5 g tube) over 20 to 25 cm2 of skin surface area) for at least 1 hour
  • Major dermal procedures (eg, more painful dermatological procedures involving a larger skin area such as split thickness skin graft harvesting): Apply 2 g per 10 cm2 of skin and allow to remain in contact with the skin for at least 2 hours.
  • Adult male genital skin (eg, pretreatment before local anesthetic infiltration): Apply 1 g per 10 cm2 to the skin surface for 15 minutes. Local anesthetic infiltration should be performed immediately after the removal of the cream.
  • Adult female genital mucous membranes: Minor procedures (eg, removal of condylomata acuminata, pretreatment for local anesthetic infiltration): Apply 5 to 10 g for 5 to 10 minutes. The local anesthetic infiltration or procedure should be performed immediately after the removal of the cream.

If your medical doctor is using this medicine to treat your pain, your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them. Do not start, stop, or change the dosage of any medicine before checking with your doctor, health care provider, or pharmacist first.

  • Lidocaine and Prilocaine Topical have severe interactions with the following drugs:
    • bupivacaine liposome
    • dofetilide
    • eliglustat
    • flibanserin
    • lomitapide
  • Lidocaine and Prilocaine Topical have serious interactions with the following drugs:
    • abametapir
    • axitinib
    • bosutinib
    • bupivacaine implant
    • cobimetinib
  • Lidocaine and Prilocaine Topical have moderate interactions with at least 78 other drugs.
  • Lidocaine and Prilocaine Topical have minor interactions with the following drugs:
    • alfentanil
    • belladonna and opium
    • buprenorphine
    • buprenorphine buccal
    • butorphanol
    • codeine
    • dextromoramide
    • diamorphine
    • difenoxin hcl
    • diphenoxylate HCl
    • dipipanone
    • hyaluronidase
    • hydromorphone

Cautions

  • Fatal overdoses have occurred when numbing medicines were used without the advice of a medical doctor (such as during a cosmetic procedure like laser hair removal). However, overdose has also occurred in women treated with a numbing medicine before having a mammography.
  • Be aware that many cosmetic procedures are performed without a medical doctor present.
  • You may need to cover the skin to help keep the medicine in place. Do not cover treated skin unless your doctor has told you to.
  • Carefully follow dosing instructions when applying this medicine to a child. Lidocaine and Prilocaine Topical doses are based on the weight in children. Do not allow a child to use this medicine without adult supervision.
  • Do not allow this medicine to come into contact with your eyes. If it does, rinse with water.
  • Do not take it by mouth. Lidocaine and Prilocaine Topical are for use only on the skin.
  • Avoid accidentally injuring treated skin areas while they are numb. Avoid coming into contact with very hot or very cold surfaces.

Pregnancy and Lactation

  • FDA pregnancy category B. Lidocaine and Prilocaine Topical are not expected to harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment.
  • Lactation
    • Lidocaine and prilocaine can pass into breast milk and may harm a nursing baby. Tell your doctor if you are breastfeeding a baby.
