What Is Sodium Thiosulfate Used For and How Does it Work?
Sodium thiosulfate is used to treat cyanide poisoning. It is also used to reduce the side effects of the cancer medication cisplatin.
Dosages of Sodium Thiosulfate
The number of sodium_thiosulfate per dose can vary greatly (from 1-8 x 1010 sodium_thiosulfate/collection) based on the donor and mobilization regimen given to the donor.
Dosage Considerations – Should be Given as Follows:
Cyanide Poisoning
- Adult:12.5 g intravenous (IV) (infuse over 10-20 minutes)
- Pediatric: 7 g/m² IV (maximum 12.5 g), infuse over at least 10 minutes
Cisplatin Extravasation
- 2 mL 1/6 molar solution through IV cannula for every 100 mg cisplatin; remove needle, then inject 0.1 mL injections clockwise around extravasation area up to 1 mL; repeat several times within the 3-4 hours of extravasation incident
Cisplatin Nephrotoxicity
- 4 g/m2 IV bolus followed by 12 g/m2 IV infusion over 6 hours
Preparation of 1/6 Molar Solution
- 4 mL of 10% solution + 6 mL sterile water for injection
- 1.6 mL of 25% solution + 8.4 mL sterile water for injection
Administration
- Monitor for thiocyanate toxicity
What Are Side Effects Associated with Using Sodium Thiosulfate?
Side effects of sodium thiosulfate include:
- Low blood pressure (hypotension) (infusion rate-dependent)
- Nausea/vomiting
- Disorientation
- Headache
- Prolonged bleeding therapy
- Hypersensitivity reactions
- Contact dermatitis
- Warmth
- Local irritation
